Temporary License- YES, additional $50, issued after receipt of completed AZBON fingerprint card, completed application, copy of current license from another state and proof of US citizenship. Finger- print card mailed to you after app received

Endorsement Cost $150 + $50 for fingerprint processing + cost of license verification, need all items listed for temp license plus verification of original state of licensure, valid for 4 years

License renewal fee $160, every 4 years by April 1st, renewable on line

Continuing education requirements - none

Comments- Walk thru available on emergency basis only, need letter from AZ employer
